Injury reporting
An injured employee, their employer, or the medical provider may report a work-related injury. The ideal process for CompManagement Health Systems (CHS) clients is to have the injured employee complete an injury reporting form as soon as possible after an injury, and then have the form faxed to CHS for reporting to the Ohio Bureau of Workers’ Compensation (BWC). The preferred BWC form is the First Report of Injury (FROI) available at
Investigation and documentation
When a work-related injury occurs, the employer should conduct an investigation and thoroughly document the incident. It is important to do this as soon as the individuals with knowledge of the incident can be contacted. Several key factors should be considered, such as ergonomics, equipment failure, weather conditions and physical capabilities.
