Injured Worker Educational Kit

CHS has developed an Injured Worker Educational Kit, Five Steps to Take if You are Injured on the Job, to assist you in the event of a Workplace Injury. The Injured Worker Educational Kit contains information about Workers' Compensation, the claims process and steps CompManagement Health System, Inc. takes to get the Injured Worker well and back to work. Also included in the folder are First Report of Injury (FROI) and Physician's Report of Work Ability (MEDCO-14) forms.
