• Status of filed claims per policy number
• Treatment plans
• Billing information, including explanation of benefits
• Physicians' request for authorization for treatment
• Accident/injury reports
• Diagnosis information, allowed and disallowed status
• Medical documents
• Physician of record
• Physician who provided initial treatment
• Return to work information
